I sent this deal to expired because even though the rebate is still valid, the digital coupon from Kroger expired yesterday so they’re no longer free.
Ran across this deal today elsewhere, then found it posted on Reddit and here. On Reddit, there’s 4 rebate forms posted, covering all 50 states:

I then responded to a post in that thread which was inquiring if it was valid in any store / liquor store:
The offer (in here) is listed as:

However, Google claims that Kroger doesn’t have a presence in these states (17 by my count):

If there’s a rebate form valid for every state, it stands to reason that they’d also be valid at non-Kroger locations (especially since there’s a solo rebate form for New York, which is among the states that apparently don’t have Kroger).

Note: Some of the 17 states may have Kroger owned affiliate (alternate brand) stores, but I’m confident some don’t even have those.
