Petco B&M store had a stack of MIR forms in front of where they stock the Litter Genie Plus.I scanned a copy of the MIR form. Notable terms and conditions:

Receive a check up to $24.99
Mail the form, UPC code, and receipt with item circled
Limit of one request per person/household
Form must be original; no copies allowed (i.e. the attached scan is for your reference only; do not print this out and mail it in)IF YOU DON’T SEE THE FORM, ask cashier or manager. they may have them.

The last time they did this there was also a PetSmart specific rebate.
I recall cashing out Petco Pals rewards buying these.
