No S&S doesn’t require 5 items, you hit the 15% discount at 5, and 5% from 1 to 4. But just one item is eligible for S&S.

I think it was Amazon Mom and now called Amazon Family, but it’s always been free to join AFAIK, and I’m not sure if you need to join at all anymore:
